Does my personal name have to be used in establishing a corporation?

It all depends on the state of formation. Some states like Delaware do not require listing the names of officers, directors, or shareholders on the articles, some other state do to this degree or another. Also, most states allow the incorporator to be a third party, so it becomes really easy to make sure your name stays off the Articles of Incorporation is many states.

You do need to list a company representative when obtaining an EIN though, however that information is not public.
